Responsibilities:
- Program Aide will monitor, observe and record patient’s current condition.
- Will notify on site care team and supervisors of any changes or unusual conditions with patients.
- Program Aid will assist patients with personal care like hygiene routines, such as dressing, bathing, using the facilities or grooming.
- Provide patients with amenities such as laundry services, housekeeping, serving meals, food shopping/ running errands and other individual requirements.
- Help educate patients on safety and proper care when using their walker, cane, eating, and taking their medicine by themselves.
- Advise patients on nutrition and always follow dietary needs and nutrition standards put in place by the doctor.
- Follow all action plans of care and contact management with any issues.
- Escorting patients outside of the facility when needed.
- Perform prescribed physical therapy services.
- Identify new and exciting recreational activities or opportunities the patients will enjoy staying active.
- Maintain a healthy, secure, and safe environment for all patients by following all safety and security precautions.
- Adhere to company standards, and all home care policies and procedures.
- High School Diploma or GED
- 2+ years of experience in home health or relevant.
- Must have experience supporting or assisting individuals with mental or physical disabilities.
- Knowledge of New York home care law, regulations, curriculum, and codes.
- Must be able to adapt to a changing environment.
- Bilingual in Spanish
- Can work Sunday-Thursday 12AM-8AM
- Ability to communicate verbally and in written form.
- Must have CPR and Crisis Certification.
- Must be vaccinated.
