Responsibilities:
- Provide direct patient care, including assessing, planning, implementing, and evaluating patient care plans
- Administer medications and treatments as prescribed by the physician
- Monitor and report changes in patient symptoms or behavior
- Collaborate with interdisciplinary healthcare team members to ensure quality patient care
- Educate patients and their families on health conditions, treatment plans, and self-care techniques
- Maintain accurate and up-to-date medical records
- Provide health coaching to patients to promote wellness and disease prevention
- Provide specialized care for patients with dementia or other cognitive impairments
- Assist with pediatric patient care as needed
- Follow infection control standards to maintain a safe and clean environment
- Minimum of 2 years of experience as a Registered Nurse in a clinical setting
- Familiarity with medical records management and documentation requirement
- Valid Registered Nurse license in the state of Oregon
- Strong communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
